If you fall into this category, you should provide your flu documentation to your company or school (unless your company or school advises you to submit your flu documentation to Ascension AOH). Any contingent worker, contractor or non-employed healthcare provider who is subject to the influenza vaccine requirement may receive a vaccine from an AOH vaccine clinic, where a receipt of administration will be provided to you.

The flu vaccine requirement applies to all employed associates, including those employed by subsidiaries and partners physicians and advanced practice providers, whether employed or independent medical students and residents volunteers and vendors and all students entering Ascension care facilities.
